Wines and Medals - What's the Significance

We've all seen the bottles of wine with a gold, silver or bronze medal sticker attached to the label. But what does that really signify? Does it mean that the wine has been entered into tasting competitions and judges voted it hands down the best, second best or bronze place finisher? If so, what was the rest of the field like?
